Imagine how frustrating it would be to know everyone was lying to you except for the people saying you are fat, which you know to be true. Situations like this make a person wonder if anyone cares, which leaves a person feeling helpless. After a while an obese person starts telling themselves the same lies, because they find denial is easier than finding a solution to their obesity.
